How to Complete Your Annual Sworn Statement

1.     You will need your Driver’s License information.

2.     You will need to have an electronic copy of your Driver’s License to upload into the system.

3.   You will need to take a picture of your Driver’s License, save the picture of your Driver's License to your computer, then upload the picture into the form.

4.   You will click the "Choose File" to upload a photocopy (aka scan copy) of your Driver's License. You will have to find where you save the photocopy (aka scan copy) of your Driver's License on your computer and attach it to the "Choose File."

PART 1 - Instruction

5.     You will need to scan or take a picture of any other permits or license from a government agency, save these documents to your computer, then upload the saved document(s) into the form.

6.     Your Driver’s License is NOT considered as “other permit(s) or license(s) from other governmental agencies.”

7.   You will click the "Choose File" to upload a photocopy (aka scan copy) of your government permit(s) or license(s). You will have to find where you save the photocopy (aka scan copy) of your government permit(s) or license(s) on your computer and attach it to the "Choose File."

8.   If you have more than one medallion, then you will need a list of all your medallions numbers.

PART 2 - Instruction

9.     You will need to know your affiliated Color Scheme.

10.     You will need your 4 digits Social Security Number (SSN).

11.     You will need your residence and/or mailing address(es).

12.     P.O. Box is NOT acceptable.

13.  You will need a residence phone. If you do not have a residence phone, then you may enter your cellular phone in the residence phone section.

Part 3 - Instruction

14.  You will need to scan or take a picture of your Driver’s license, and save the picture of your Driver's License to your computer, then upload the picture into the form.

15.  You will click the “Choose File” to upload a photocopy (aka scan copy) of your Driver’s License. You will have to find where you save the photocopy (aka scan copy) of your Driver’s License on your computer and attach it to the “Choose File.”
